It is with regrets that I have decided to retire from Family Practice, as of February 5, 2012.
杨棣錝家庭医生 (DR. JOHN T. YANG) 将于2012年2月5日退休。
In order to receive your copies of Medical Record, please call 647-8888 and make an appointment to come to the office in person before February 4, 2012. You must bring your Medicare card as your I.D. and sign a form before the record is discharged. (Please call 7 days in advance and do not wait until the last week to pick up your chart).
请在2012年2月4日前办理完提取个人病历本等相关事宜。请提前电话预约(647-8888) 提取时间，必须带上医疗卡，本人亲自到办公室办理。(请提前7天打电话，不要等到最后一个星期才来取。)
*Since the medical records are confidential documents, if you want to pick up your
spouse’s or your friend’s chart, you must bring in his/hers signed consent letter.
All the information has to be the same on the Medicare I.D.
Sample of the consent letter:
*由于个人病历本属保密文件，所以，如你的配偶或朋友需要委托办理，你必须带
上他（她）的签名的授权书。所有在授权书上的资料必须与医疗卡的相同。

*If you are a minor under 17 year-old, your parents could pick up the medical record
for you. Also, your parents have to show your Medicare I.D. in order to receive your
record.

*After the date of February 5, 2012. All the medical records will be transferred to a
medical record storage company. Patients who pick up their medical records after the
due date have to directly contact this number 1-800-775-0093,
Record Management Co., Toronto, and there will be a charge for the service.

Since there is shortage of physicians in Saint John and all over Canada, you may have to make an effort to seek one yourself.
These are the steps you can do:

1. Keep your eyes and ears open for a new physician coming in town.

2. Call 648-6232, Atlantic Health Sciences Corporation and leave your name and telephone number. They will call you when a family doctor becomes available.

3. If you feel it is an Emergency problem (e.g. chest pain…) you should call an ambulance at 911 and go to Emergency Department of Saint John Regional Hospital.

4. In the meantime, if you have an urgent medical problem, you can go to St.
Joseph’s Hospital “Urgent Care walk–in” service. It is opened daily from 9:00am
to 9:00 pm. You have to register and the last patient registration is at 8:00pm.

It is the number for Tele-Care and access to registered nurses for non-urgent health advice. This service is available 24 hours a day, seven days a week.
